The National Patient Safety Agency states that ‘Trusts that report high levels of patient safety incidents suggest a stronger organisational culture of safety because they take all incidents seriously and link reporting with learning.’

Incident Reporting is the system of identifying and acknowledgment of unplanned events that could have or did result in harm, injury or service and financial loss in an organisation. This masterclass explores the core components of effective incident reporting.
