Ensuring the support services and infrastructure are in place to support a 7 day NHS service model

Abstract

Implementing a 7 Day Consultant Radiology Service – The West Herts Experience

Traditionally radiology departments have operated most of their services on a Monday to Friday, 9-5 model. Outside of these hours plain film radiology support to acute care has been available and an emergency service with radiologists on-call from off-site for complex imaging and intervention. Increasingly this model is being recognised as inadequate. In a society in which other services are available every day it is becoming increasingly incongruous that only emergency healthcare is available in evenings and at weekends. Extended provision of services has clearly demonstrable advantages to the quality of healthcare as well as allowing for more efficient use of valuable resources and being more responsive to patient’s needs. Such changes to diagnostic services should not, however, be planned in isolation from other services and should be considered as part of a holistic approach to the provision of healthcare.
 
In this talk I will discuss these issues with reference to the Department of Health guidance issued last year. I will describe our own experience of providing an extended hour Consultant Radiology service in acute care, based around a dedicated radiology facility within a very large purpose built Acute Admissions Unit. I will explain how this service was developed and how it operates. I will also discuss how we have used this opportunity to make wide ranging improvements in the way that Radiologists work.

Dr Danny Boxer 
I am a Consultant Radiologist based at Watford General Hospital, part of West Hertfordshire NHS Trust. After gaining MRCP(UK) I trained in Radiology at The Westminster, Central Middlesex and St. Mary’s Hospitals, obtaining FRCR in 1989 and becoming a consultant in 1992.

I consider myself to be a General Radiologist with an interest in axial imaging, particularly ultrasound. I have particular commitments to gynaecological, urological and paediatric imaging and plain film work.

I have a long-standing interest in management. Following a stint of 4 years as Clinical Director I became an Associate Medical Director for Service Modernisation and then Clinical Champion for Diagnostic Workstream Modernisation. These latter two posts involved leading strategic planning for the directorate and implementing the modernisation agenda. I am a technical assessor for the national Imaging Services Accreditation Scheme.

In 2009 the Trust centralised acute services and opened what is believed to be the largest purpose built Acute Admissions Unit in Europe. I led the planning for the provision of radiological services and consequent modernisation of Radiologist’s working practices. I was a member of the Department of Health working party on the provision of extended hours working in Radiology. I lecture widely on the subjects of modernisation of working practices and the provision of extended hours radiology services. I act as a clinical advisor to Xpand Solutions Ltd.
