Masterclass: Discharge Planning with a Difference including Nurse Led Discharge

This Masterclass will examine the elements of safe, effective patient discharge from hospital. The evidence base together with practical tools and suggestions will form the framework for the day.
This masterclass will benefit delegates by providing the opportunity to revisit the discharge process within your organisation with a view to stimulating new and innovative approaches for effective patient discharge. The evidence base and practical tools to take away, will underpin the topics of risk, safety and patient centeredness.
It begins by providing an overview of national UK discharge policies/best practice guidance and encourages delegates to highlight the significance of these to their practice. Delegates will be able to explore a deep understanding of risk, patient assessment and patient centeredness for discharge, to critically evaluate how and why aspects from this should be considered in practice settings. Finally, criteria led discharge and its implementation will be described (with tools).
The day will be interspersed with plenty of opportunities for discussion in order to challenge issues, which are considered barriers to a smooth patient discharge process.
- To review the elements of the patient discharge process, through mapping: to enable any gaps and overlaps to be reconsidered in the context of policy/best practice and the evidence base.
- To explore the antecedents, critical attributes and consequences for patient risk assessment, through examination of the pertinent literature: to demonstrate useful tools for this purpose in the process of patient discharge from hospital.
- To engender a patient centred discharge process through discussion of current research findings: to stimulate new ideas and approaches to an age-old problem.
- To analyse factors of safety, barriers /facilitators, readmissions and process for patient selection for discharge (criteria-led discharge) through group work: to enable core considerations in establishing a process.
