Enhanced health in care homes vanguards

There are six enhanced health in care home vanguards offering older people better, joined up health, care and rehabilitation services.

These vanguards were selected following a rigorous process, involving workshops and the engagement of key partners and patient representative groups.

  4. Nottingham City Clinical Commissioning Group
  5. Sutton Homes of Care
  6. Airedale & Partners

Enhanced health in care homes framework

The enhanced health in care homes (EHCH) framework (published 29 September 2016) lays out a clear vision for working with care homes to provide joined up primary, community and secondary, social care to residents of care and nursing homes, via a range of in reach services.

Working together for better care – learning from enhanced health in care home vanguards

Watch a short film about the work and achievements of the six enhanced health in care home vanguards and their story so far. Through a series of interviews with staff, residents and volunteers, the film showcases the impact that the new models of care are having on residents health and wellbeing and how vanguards are achieving this.
