This national virtual summit focuses on supporting staff to deliver positive patient identification as well as implementation and adoption of the recommendations from the HSSIB Positive Patient Identification report.

Through national updates, practical case studies and in depth expert sessions the conference aims to improve the effectiveness of patient identification within your service, and ensure that new technology and optimisations lead to change and improvements in patient care. The conference will also reflect on the staff training needs for positive patient identification, as well as evaluating the current systems and technology in clinical practice.

Responses to Freedom of Information requests from 166 trusts revealed 58,537 cases of patient misidentification logged in Datix or other patient safety systems between 2019 and 2023, including 4,713 causing some sort of harm. This is equivalent to 11,707 incidents a year, with 943 leading to harm.”
